Retinal detachment

Separation of RPE (Retinal pigmented epithelium) from neurosensory retina.

Retinal detachment (RD)

Description

Examination

finding

Rhegmatogenous RD

(Rhegma = Break)

Retina continuity break

Tear (due to traction)-

• Horseshoe shaped RD

• M/C cause is PVD

• PVD is physiological (normal) in old age

Hole (after degeneration)-

• Lattice degeneration

• Snail track degeneration

• Corrugated appearance

• Schaffer’s sign

/ Tobacco dust

• Undulating movement

• Photopsia

• Floaters

Tractional RD

(In vascular disease)

• Concave RD

• Tractional bands

• Immobile

• Photopsia + / –

• Floaters absent

Exudative / Serous RD

(Due to choroid pathology)

Causes:-

• Inflammation – Posterior

scleritis

• Vascular – Central serous

chorioretinopathy

• Neovasculaziation – Wet ARD

• Tumor

Systemic-

• Renal hypertension

• Pregnancy induced

HTN (PIH)

• Convex RD

• Mobile (due to shifting fluid)

• Leopard spots (pigment deposition in

subretinal space)

• No photophobia

• Floaters + / –

PVD- Posterior vitreous detachment
RD- Retinal detachment

HTN- Hypertension
